Project Overview

The Community Health Resource Project (CHRP) is collaborating with rural and underserved counties in Southeast Georgia to identify and map health resources that promote positive health outcomes. Health resources aren’t just your doctor and dentist, they are also parks and recreation, healthy food, housing, senior services, education, environmental services, healthcare, daycare, disability services, animal/veterinary services, transportation, social services, public safety, and agricultural services. In addition, the data we are collecting for each partner county can help support health workforce development.

Prepare, Engage, Report (PER)

Preparation Phase: Team members conduct an environmental scan of each county, identifying leading health indicators and compiling demographic and geographic data. They also identify organizations promoting positive health based on social determinants like economic stability, education, healthcare access, neighborhood environment, and social context.
Engagement Phase: The team sends introductory emails, conducts windshield and walking tours with drop-in conversations, and schedules formal meetings with community members and leaders to gather information on health assets. They also attend community events, like farmers’ markets, to build trust and relationships.
Reporting Phase: An asset map and detailed county reports are created. These reports outline how counties utilize their resources to meet health needs. Each county partner will receive their specific report and access to the health asset map.

Project Reach

In the first year, CHRP is focused on community outreach and relationship building, aiming to create six county-specific community asset (resource) maps and reports. In each county, a strengths-based approach is used to highlight existing resources and services that contribute to positive health outcomes.
